论证。附骨切除术用于矫正儿童下肢不等长的问题。关于使用8型钢板进行附骨切除术对这种病症的疗效,研究结果并不明确。对矫正的预测是使用附骨切除技术的基本条件。
目的。本研究的目的是根据患者的年龄和骺板截面,确定使用8型钢板临时附骨切除术矫正儿童下肢长度不等的相对有效性。
材料与方法。该研究包括对94名患者使用控制生长法矫正下肢不等长结果的回顾性分析数据。手术治疗方法包括使用两块8型钢板。计算了相对指标,确定了骨骺分离前和治疗结束时骨骺分离和成对完整节段长度的百分比差异(去除金属结构),作为治疗有效性的指标。
结果。年龄较小的儿童进行股骨外固定术的效率最高(骨长变化率为7.59%),而年龄较大的儿童进行胫骨外固定术的效率最低(骨长变化率为2.04%)。股骨骨骺成形术的效率随年龄呈线性下降,年轻组与中年组的效率指数相差1.85%,中低年级组疗效差异为1.85%,中高年级组疗效差异为2.8%。在胫骨外固定术中,年轻组和中年组的有效率差异为2.42%,中年组和老年组的有效率差异为0.34%。
结论。使用8型钢板进行临时骨外固定术可有效矫正儿童下肢长度不等的问题。利用我们获得的不同年龄段的相对疗效指标,可以更准确地计划干预,避免不合理的手术。
