低龄儿童斜颈的诊断方法研究

论证。斜颈(torticollis)是头部和颈部恶性位置的一个常见术语。斜颈可能是各种病理过程的结果,从相对良性到危及生命。这种综合征在儿科实践中具有特殊的相关性,在初级保健水平上经常被低估。
目的是分析国内外文献数据,反映儿童各种类型斜颈的病因和临床特点,并开发方法,以鉴别诊断在较年轻年龄组的患者。
材料与方法。利用关键词和短语在开放信息数据库еLIBRARY和Pubmed中进行文献检索:“斜颈”、 “先天性肌性斜颈”、“非肌性斜颈”、“获得性斜颈”、“神经性斜颈”(torticollis, congenital muscular torticollis, nonmuscular torticollis, acquired torticollis, neurogenic torticollis),但不限制回顾的深度。
结果。根据文献资料,以表格形式给出斜颈的分类及鉴别诊断的重点方向。斜颈的鉴别诊断范围是相当广泛的,在一岁的儿童有自己的特点,不像较大的儿童。最常见的是先天性肌性斜颈。与此同时,非肌性斜颈也很常见,其特点往往是更严重的病因,在这种情况下,需要更彻底的检查。在这项研究中,我们已经编制了一致的方法,鉴别诊断斜颈的儿童更年轻的年龄组。
结论。提高儿科临床医生对斜颈综合征病因的认识水平,将提高早期诊断导致儿童病理性头颈部安装的危险疾病的有效性。
