肠旋转不良的并发症最常出现在生命的头几个月,但也有在晚年诊断的病例,包括在成人中。临床情况变化很大,通常需要很长时间才能做出正确的诊断。报告一例17岁女孩因孤立性十二指肠旋转不良及过度固定所致慢性十二指肠梗阻的临床观察。考虑到患者的合并症和精神发育障碍,诊断搜索非常困难。这种情况下的手术干预涉及十二指肠的动员和病理性固定的消除。然而,考虑到十二指肠壁的明显变化,结合病理,决定进行十二指肠空肠吻合术。尽管进行了额外的手术入院,但术后的特点是胃十二指肠淤滞长期存在。充足的饮食、混合营养、最大限度的“垂直化”和患者的激活对于阻止术后吻合口炎和运动障碍的发生起着关键作用。因此,在治疗一名患有手术、治疗病理和精神障碍复合体的青少年时,严格个体化的方法取得了令人满意的治疗结果。
