利用生物反馈现代设备技术(HandTutor)对缺血性脑卒中患者肌肉力量和肌肉张力的正常化研究

背景。急性脑血流障碍的广泛流行性常伴随上肢功能的持久损害,导致长期丧失劳动能力。这一医学和社会问题的严重性促使我们寻求新的神经康复方案。
研究目的。研究结合生物反馈(Biofeedback,BFB)的设备方法与功能性可编程电刺激和磁刺激对缺血性脑卒中患者肌肉张力的影响特点。
材料与方法。本研究纳入了117名缺血性脑卒中患者,病程为3天至12个月,年龄在50至75岁之间(平均年龄为61.2±3.4岁),诊断依据《国际疾病分类第10版》(ICD-10)明确为“卒中后状态”。患者中男性68人(58.1%),女性49人(41.9%),随机分为4组,根据非药物基础治疗背景下不同方法的组合进行分组:第1组(对照组—功能性可编程电刺激)- 28名患者,进行结合功能性可编程电刺激的精细运动功能恢复措施;第2组(对照组—磁刺激)- 30名患者,接受结合磁刺激的精细运动功能恢复措施;第3组(BFB + 功能性可编程电刺激)- 32名患者,使用带有生物反馈功能的设备(HandTutor)与功能性可编程电刺激相结合的康复措施; 第4组(BFB + 磁刺激)- 27名患者,采用带有生物反馈功能的设备(HandTutor)与磁刺激相结合的康复措施。
结果。四组患者的康复治疗均促进了肌肉力量的提高和肌肉张力的正常化。与对照组相比,使用基于生物反馈原理的HandTutor设备进行的康复措施显著改善了患者的肌肉力量和肌肉张力:康复疗程结束后,肌肉力量提高了17.7%–22.3%(12个月后为22.7%–31.6%),肌肉张力(根据痉挛评估量表)降低了21.3%–48.4%(12个月后为21.3%–57.3%)。
结论。将生物反馈技术与功能性可编程电刺激和磁刺激相结合,纳入卒中后上肢精细运动功能受损患者的康复治疗方案,可显著改善患者的肌肉力量和肌肉张力。
