Cutaneous Endometriosis
Abstract
Cutaneous endometriosis is a pathological process with benign growth of tissue, morphologically andfunctionally similar to the endometrium. Endometriosis develops in tissues and organs of females and is presented by solitarty or multiple nodular formations; its most incident location are the pelvic region and the genitals. Cutaneous endometriosis is responsible for 0.42-4% of all endometriosis variants. The development of endometriosis in the skin and external genital mucosa is assumed to be caused by the hematogenic and presumably lymphogenic transport of the uterine endometrial cells. In addition to the vascular transplantation of the uterine endometrium, mechanical transfer is posible, as well as implantation during surgical interventions (cesarean section, salpingectomy, appendectomy, laparotomy, herniotomy). A clinical observation is presented.

Kaposi's sarcoma: pathogenesis, clinical picture, diagnosis, and modern therapy
Abstract
The results of epidemiological studies indicate an increase in the incidence of Kaposi’s sarcoma in recent years. The disease more often develops in subjects aged over 60, though the mean age of the patients reduced significantly in recent years. This is attributed to untoward socio-ecological factors, promoting the development of immune disorders and the relevant attenuation of the antitumor immune surveillance function. Kaposi’s sarcoma is a serious multidisciplinary problem; dermatologists, oncologists, and physicians of other specializations have to face it. This paper presents analysis of modern concepts of the pathogenesis, diagnosis, and therapy of Kaposi’s sarcoma. Published data indicate that photodynamic therapy induces the efficiency of therapy for Kaposi’s sarcoma. Medical laser technologies, including laser-induced thermotherapy, are now often used in dermatology. The efficiency of this method in cutaneous diseases of different etiology (including basal-cell carcinoma) has been demonstrated not once. The problem of developing effective methods for the treatment of Kaposi’s sarcoma remains unsolved, as all the known methods do not lead to complete cure, but just to temporary inhibition of the process without preventing its relapses and further progress. Analysis of published data indicates that today many specialists emphasize the importance of a differentiated approach to therapy, depending on the disease type and form, stage, dissemination of the process, immunosuppression degree, drug sensitivity, and severity of side effects of therapies. Use of this approach leads to effective control of the tumor process in the majority of cases, often with resolution of the majority of efflorescence and with lasting remissions.

A rare benign tumor in a child
Abstract
Published data on the pathogenesis and diagnosis of a rare benign tumor - pilomatrixoma - are presented. The authors present information on the tumor etiology and the numerous theories on its pathogenesis; however, the data indicate that the tumor consists of cells of the external membrane of the hair follicle root, with a trend to keratinization. The tumor is liable to form relapses after removal. Invasive growth and malignant transformation by the basal cell type can develop in these cases. A clinical case is presented: a child aged 9 years with pilomatrixoma. The tumor was surgically removed, the diagnosis was made on the basis of histological findings. Layers of necrotic cells, enveloped in a fibrous capsule, were detected, with cellular infiltration from lymnphocytes, histiocytes, and multinuclear cells and with focalfibrosis between the layers.

Clinical immunological aspects of lichen planus development
Abstract
The clinical course of lichen planus (LP) was studied and the contribution of disorders in interleukine-dependent modulation of congenital and acquired immune response to the development of this dermatosis was evaluated. Twenty-eight patients with clinical variants of LP were observed. Differences in the clinical picture of LP in comparison with the classical descriptions were detected and disproportions in the system of interleukines, interferons, and some adhesion molecules were evaluated. Herpes simplex virus was found to be one of the factors triggering the development of LP.

Anetodermia in practical dermatology
Abstract
A rare disease of connective tissue is presented - anetodermia, a dermatosis of unknown etiology, leading to destruction of elastic fibrils and skin atrophy in the foci. Historical data on the dermatosis and its probable causes are discussed, clinical variants of anetodermia, differential diagnosis, and basic drug, topical, and spa therapies are described. The authors present three clinical observations of Jadasson’s, Schwenninger-Buzzi’s, and secondary anetodermias after varicella in patients of different age. These cases demonstrate a variety of clinical manifestations of spotted skin atrophy in patients of quite different age and the differences in the case histories.

Fish peeling aggravated by panniculitis in a female tourist after voyage to Thailand
Abstract
Skin infection with the development ofpanniculitis in a female patient after a course of ichthyotherapy (fish pedicure/peeling) in Thailand is described for the first time in Russian literature. Fish bacterial agents, which can infect the skin of individuals visiting the fish spa, are discussed. The risk of infections of the skin is higher for tourists visiting tropical countries if these tourists suffer from concomitant diseases attenuating the immunity and associated with blood clotting disorders. Methods for prevention of complications of ichthyotherapy and the relevant disinfection and sterilization problems are discussed.

Pathogenetic role of lipid profile disorders in psoriasis
Abstract
The pathogenetic role of metabolic disorders, specifically, changes in the lipid profile in psoriasis is discussed. Analysis of published data indicates that metabolic disorders are significant pathogenetic factors for the psoriatic process, some of them triggering the process; this determines the importance of the problem and suggests further research in this direction.

Matrix metalloproteinase 9 is a marker of chronic inflammation in cutaneous vasculitis
Abstract
The morphohistological picture of skin biopsy specimens from patients with cutaneous vasculitis is presented. Dilatation of dermal vessels and increase of their numbers, increase of perivascular lymphocytic infiltration of the derma in comparison with the parameters in biopsy specimens of the skin from healthy volunteers indicates neoangiogenesis and pathological dilatation of the dermal capillaries. High concentrations of matrix metalloproteinase 9 (MMP-9) and its homogenous dissemination in the specimens from patients with dermal vasculitis presumably indicate the regulatory role of MMP in extracellular matrix remodeling and neoangiogenesis.

On therapy of surface pyoderma
Abstract
Classification and basic approaches to therapy of superficial forms of pyoderma, including therapy with sulfathioasole silver salt, are presented. Argosulfan drug is presented and the mechanism of silver sulfathiasole activity towards suppurative lesions of the skin are described. Clinical efficiency of Argosulfan cream in therapy of various forms of surface pyodermas is shown.

Linear IgA-dependent bullous dermatosis
Abstract
Published data on linear IgA-dependent bullous dermatosis - modern data on the etiology, pathogenesis, and therapy are presented. The authors present their observation of a patient with linear IgA-dependent bullous dermatosis. The clinical picture is described in detail; the disease was diagnosed by standard and other effective noninvasive methods.

Chronic urticaria: diagnostic approach practiced by medical specialists and general practitioners of Russia
Abstract
Chronic urticaria (CU) is a highly prevalent cutaneous disease, characterized by almost daily emergence of blisters, itching, sometimes angioedemas (AE), persisting during more than 6 weeks. Thi aim of this study was to evaluate the knowledge of the diagnostic methods used in CU by medical specialists (allergologists, dermatologists) and general practitioners (pediatricians, therapeutists) from various cities by evaluating their knowledge of the international and national clinical recommendations and consensus documents. The study was carried out in 1500 physicians (350 allergologists and immunologists, 500 dermatologists, 300 pediatricians, and 350 therapeutists). After rejecting the defective questionnairs (unfit for processing), 300 questionnairs were included in the study, responded by 60 (20%) allergologists-immunologists, 108 (36%) dermatologists, 61 (20.3%) pediatricians, and 71 (23.7%) therapeutists. Of all the respondents, 42.7% knew the national and foreign clinical recommendations on the diagnosis of CU. Allergologists were 4.9 times more informed than therapeutists, 2.9 times more than pediatricians, and 2.3 times more than dermatologoists (x 2 1 = 76.110; p < 0.0001; V = 0.504). Physicians who did not know the consensus documents - mainly the primary health care physicians (therapeutists, pediatricians) - were liable to nonoptimal diagnosis and consulted their colleagues of allied specialization 2 times more often (x 2 1 = 30.516; p < 0.0001; V = 0.403). This could be attributed to the fact that they did not know about the existence of the recommendations, had no time to study these documemnts, their foreign languages level was low, and some analysis were unavailable for therapeutic institutions. Physicians can be better informed by organization of allergology schools, round tables, master classes, lectures, and via internet training (web workshops, presentation of modern manuals at specialized sites, education programms in social networks).

Review of pathogenetic mechanisms and methods for scar correction of cicatrices
Abstract
The choice of an adequate methodfor the treatment of scar remains a challogingproblem. The pathogenesis of this process is still not known except that genetic liability, hyperproduction of growth factors, specifically, of transforming growth factor ft, imbalance between matrix metalloproteinases and their inhibitors are involved in scar formation. Drug therapy, spa therapy, and surgery are now usedfor correction scar; however, despite the numerous relevant methods, the problem remains unsolved.

Clinical efficiency of Zineryt lotion and its effect on the quality of life of adolescents with mild and medium severe acne
Abstract
Modern literature on the pathogenesis of acne is reviewed. The role of Propionbacterium acnes in stimulation and maintenance of immunological mechanisms of disease emergence and maintenance is emphasized. The effects of acne on the mental status and quality of life of patients of different age were studied. The effects of acne and Zineryt lotion therapy on the mental status and the relevant quality of life of the youngest and most vulnerable patients were evaluated within the framework of a SPOTLESS multicenter prospective observation program, aimed at evaluating the clinical efficiency of Zinerit lotion and its effects on the quality of life of adolescents with mild and medium severe acne. The treatment efficiency was evaluated by the time course of inflammation elements and by the effects of acne and therapy on the patients ’ quality of life.

Phagedenic chancre: a case history
Abstract
A clinical case is presented: a patient (male) aged 26 with malignant syphilis, presented by phagedenic degeneration of the hard chancre, which is difficult to diagnose. Blood testing for syphilis showed positive reactions. Primary syphilis of the genitals and syphilitic chancre (A51.0) were diagnosed. Careless sexual contacts and alcohol abuse promoted a severe course of the syphilitic infection. Therapy led to clinical serological cure. This case is presented to attract the attention of dermatologists and venereologists and of physicians of allied specializations, e. g. urologists, to the problem of syphilis.
