浅谈幼儿髋关节发育不良骨盆截骨技术的选择问题

论证。在幼儿髋关节发育不良的晚期诊断中,骨盆截骨术的选择通常取决于外科医生的经验和偏好。对畸形程度的诊断是基于普遍接受的髋关节发育不良分类,而不考虑髋臼畸形的可能变体。我们提出的假设是,在对不同严重程度的髋关节发育不良患儿进行手术治疗时,骨盆截骨技术的选择应取决于髋臼畸形的情况和截骨术本身的矫正能力。
目的。本研究的目的是对不同严重程度的髋关节发育不良患儿手术治疗结果进行比较分析,评估在选择骨盆截骨技术方面所建议的区别对待方法的有效性。
方法。该研究包括150名患者(150个髋关节),年龄范围在2至4岁之间(3.1±0.45),根据D. Tönis的增强分类法,髋关节发育不良的严重程度为II至IV级。根据已正是的髋臼畸形类型,并考虑到不同类型截骨术的矫正能力,患者被分为三组。所有患者都接受了常规的临床和放射检查。放射测量法评估了以下参数:髋臼指数、角、颈干角、股骨近端内翻角、骨覆盖程度、髋臼深度和骨盆高度、髋臼穹隆范围以及有无骨性直立器。
结果。对不同严重程度的髋关节发育不良患儿的髋关节X射线解剖状态进行的比较分析表明,有区别地应用无自体移植的改良Salter骨盆截骨术、囊周髋臼成形术,Pemberton和Pembersal手术可对各种先天性髋臼畸形进行适当矫正。此外,这种技术还能提供接近正常值的髋臼解剖结构,并且不会导致半骨盆出现明显的伸长变形。
结论。在对患有不同程度髋关节发育不良的幼儿进行治疗时,根据髋臼畸形的变异情况,采用差异化的方法选择骨盆截骨技术,可以充分矫正关节髋臼部分的先天性畸形,恢复其解剖结构,避免手术半骨盆侧形成继发性畸形。髋臼指数、Wiberg角、髋臼深度和骨盆高度的变化证实了所建议方法的有效性,其数值接近个体标准(p>0.05),并消除了可能出现的继发性畸形。
