论证。儿童股骨近端畸形是最难进行手术矫正的病症之一。大多数情况下,股骨截骨术都是在不同平面上进行的,但近几十年来,随着股骨近端生长区管理的发展,股骨近端畸形矫正术(epiphysiodesis technique)已经可以实现微创矫正。
本研究旨在分析国外和国内文献中反映股骨近端生长区外固定技术发展阶段的数据,以及该技术在髋关节病变儿科患者中的应用结果。
材料和方法。本文研究了世界文献中对儿童股骨近端生长区进行微创骨外固定手术治疗的结果。该研究在开放式电子科学文献数据库eLibrary和PubMed中进行了搜索,时间跨度为1933年至2022年。
结果。多数作者报告了儿童股骨近端畸形良好、满意的矫正效果,并提出了通过及时实施骨痂手术预防这些畸形发展的建议。然而,关于手术干预的时机和方法仍未达成共识。
结论。儿童股骨近端畸形,如股骨颈外翻畸形及其复发、卡拉姆奇II型无血管坏死的后遗症以及股骨大转子肥大,长期以来都是通过严重的手术干预--股骨矫正截骨术--来矫正的。检查方法的改进和对股骨近端生长区功能的更深入了解使骨科医生能够采用侵入性小、创伤小但同样有效的方法,通过有控制地阻断生长区来纠正这些股骨近端畸形。
