排除万难:环撕脱伤后的创伤性拇指截肢

背景。即使是对经验丰富的显微外科医生而言,撕脱性断指再植也常常是一项外科挑战。因此,对于环撕脱后的创伤性拇指截肢,往往难以在完全截止和手指再植之间选择治疗
途径。本文讨论了一位优势手拇指不幸被截肢的儿童患者病例中所遇到的外科挑战和使用的
管理策略。
临床病例。本文展示的一项病例中,一名十岁男孩的右手拇指在环撕脱伤后被完全截断,在之后的不同阶段采用了多种方法和手术进行重建以解决不同困难。我们重新植入撕脱的拇指,重建肌腱,使用同种异体皮肤和局部皮瓣覆盖软组织,并改善了手术方法来克服术中和术后并发症,如动脉吻合术导致的血栓形成和静脉淤血。
讨论。关于拇指环撕脱伤的治疗文献很少。据文本作者了解,仅有一例儿童病例报道。本文所描述的病例中,我们报为了补救拇指(特别在儿科群体中),排除万难,进行了复杂的重建手术,并取得了良好的结果。在术后重建方面,该男孩在功能和外观上都取得了满意的效果。
讨论。环撕脱伤后的断指再植对显微外科医生和患者来说都具有各种挑战。手术策略必须根据知识、经验和良好的显微外科技术进行调整并量身定制手术方法。在儿科人群中,即使是复杂的损伤,只要仔细计划并对手术前期的每个并发症进行适当干预,也可获得良好的手术结果。
