儿童肱骨内上髁骨折伴关节内肘部卡压的切开复位术和克氏针固定

背景。内上髁骨折(MEF)在儿童和青少年肘关节骨折中十分常见,常伴有肘关节脱位。传统的石膏固定正逐渐被早期切开复位术和克氏针或螺钉固定所取代。目前在医学文献中缺少关于正确治疗MEF的共识。
目的。本研究目的是报告经切开复位术和克氏针固定治疗的MEF伴关节内碎片嵌顿患者的临床和影像学结果及并发症。
材料与方法。回顾性分析13例(8至13岁)内上髁骨折(MEF)伴关节内肘部卡压的儿童。所有入组患者均行手术治疗,均采用切开复位术和克氏针固定,未探查尺神经。采用上肢额位对齐、肘关节活
动度(ROM)、Mayo肘关节功能评分(MEPS)和视觉模拟评分(VAS)评估临床结果。同时评估放射学结果和并发症。
结果。在平均24.1个月的随访中,患者均未表现出上肢轴向畸形或伴随ROM受限的肘关节不稳定。
MEPS平均值为98.8,VAS评分平均值为1。最终的X光显示11例患者骨折愈合,2例(15.3%)报告无症状骨
不连。13例患者中有6例出现尺神经区域术前感觉异常,但所有患者在平均4.3个月后完全恢复。所有患者在术后平均5.4个月后恢复体育运动。1例患者(7.7%)报告发生浅表外科伤口感染,接受口服抗生素治疗,无进一步手术。未发现其他并发症。
结论。结果表明,不进行尺神经探查,采用切开复位术和克氏针固定治疗MEF伴肘关节内卡压是一种安全有效的手术方法。
