INFLAMMATORY LINEAR VERRUCOUS EPIDERMAL NAEVUS (ILVEN)

摘要

Etiopathogenic factors of ILVEN occurrence are considered in the article. There are clinical and morphological criteria to verify this pathology, such as: beginning of the disease at an early age, women predominance among the cases, the presence of linear rash resembling psoriasis, skin itching, frequent rash localization in the left lower limb, resistance to treatment, and histological features, which combine features of verrucous epidermal nevus with eczematization phenomena in the epidermis or psoriaziformnymi changes. The article describes clinical case of unilateral linear warty rash for pregnant women, which illustrated the relationship of cutaneous manifestations pathological process and its further course in connection with pregnancy.

摘要

А clinical case of a rare form of pustular psoriasis, known as “acrodermatitis continua suppurativa Hallopeau” is described. The involvement of the distal phalanges of the fingers and toes in combination with localized psoriasis vulgaris is observed. Specific features of this case are the presence of two different forms of the disease and comorbidities, that limits treatment options. The article provides current data on the clinical features and management of acrodermamtitis continua suppurativa Hallopeau. An acceptable personalized treatment with systemic retinoids and topical substances containing betamethasone dipropionate and calcipotriol has been proposed.

摘要

The aim. investigation of the possibilities of dermatoscopy as a method for evaluating the effectiveness of therapy with topical imiquimod in patients with superficial basal cell carcinoma (CCB). Materials and methods. the study included 24 patients with a superficial form of CCB, who for various reasons did not undergo excisional surgical intervention and who agreed to alternative drug therapy (officinal original topical imiquimod). Patients were divided into two groups, in which the efficacy of therapy was assessed either by clinical or by dermatoscopic signs. General results. the study demonstrated the importance of dermatoscopy as a method for assessing the effectiveness of therapy with topical imiquimod; Greater prognostic significance of dermatoscopy in comparison with a simple clinical examination of mass when assessing the moment of interruption of drug therapy with imiquimod.

MILKER’S NODULE. A PERPLEXING FARMYARD INFECTION AND THREAT TO THE IMMUNOCOMPROMISED

摘要

Milker’s nodules, also called paravaccinia, is a DNA virus transmitted from infected cows to humans. It results from contact with cattle, cattle byproducts, or fomites. Classified as an occupational disorder, those at risk of exposure include farmers, butchers, and agricultural tourists. The viral infection begins 5—15 days after inoculation as an erythematous-purple, round nodule with a clear depressed center, and a surrounding erythematous ring. While familiar to those in farming communities, the presence of the nodule may be concerning to others, particularly the immunosuppressed. Milker’s nodules are selflimited in immunocompetent individuals and heal without scarring within 8 weeks. Another member of the Parapoxvirus genus, the orf virus, is also transmitted from animals to humans by direct-contact. While complications are rare, hematopoietic stem cell transplant recipients are at risk of graft-versus-host disease, as the parapoxvirus may trigger these complications in immunocompromised individuals. In addition, paravaccinia may serve as the antigen source for the development of erythema multiforme. The unique structure and replication process of viruses in the Poxvirus family, while includes the Parapoxvirus genus, have been a focus for treatment of infections and cancer. Manipulation of these viruses has demonstrated promising therapeutic possibilities as vectors for vaccines and oncologic therapy.

AUTOIMMUNE BULLOUS DERMATOSES. DIFFERENTIAL DIAGNOSIS

摘要

The review presents modern ideas concerning autoimmune bullous dermatoses (pemphigoid group): Duhring disease, bullous pemphigoid, cicatrizing pemphigus, pemphigoid gestationis, linear IgA bullous dermatosis and acquired bullous epidermolysis. Pathogenesis, clinical manifestations and modern methods of disease diagnostics are reported. Particular attention is paid to the diagnosis of acquired bullous epidermolysis, in which the leading role belongs to the reaction of immunofluorescence. The pathogenetic similarity between bullous pemphigoid, linear IgA dermatosis, cicatrizing pemphigoid and pemphigoid gestationis is shown, which allows us to regard them as a variety of a single pathological process.

The review touches upon a problem of coinfection caused by several important fungal infections (aspergillus, candida, pneumocystis, cryptococcus) and infectious or parasitic diseases. The authors also investigate epidemiologic determinants of such clinical forms of pathologic process. There are a lot of scientific gaps in the studying coinfections concerning mycosis and other infections. Physicians and infection control practitioners sometimes reveal such cases therefore complex characteristics of the coinfections is required.

Actuality. High incidence of atopic dermatitis and its association with systemic pathology determine the need of broad treatment options on the base of the modern data on pathophysiology of the disease. Aim of the study. Analysis of the clinical efficacy of the complex therapy with application drug Kestine® and probiotic FlorOK. Materials and Methods of the study. 55 patients with atopic dermatitis with mild and medium severity stages were examined and treated. The SCORAD Index and the Prurindex were used for assessment of AD severity; H2- test was used for evaluation of the digestive system function. Study results. It was stated high clinical efficacy of the complex therapy with application drug Kestine® and probiotic FlorOK accompanied by fast arrest of the itch.

PATHOMIMIA IN CHILDREN

摘要

The article is dedicated to the problem of children pathomimia. Autoaggressive injuries in pediatrics are very common; they have a variety of clinical manifestations and can be a symptom of severe mental disorders. The article describes in detail the mechanisms for the formation of autoaggressive behavior, provides a classification of psychodermatoses, and describes the clinical features of the course of pathomimia in childhood and adolescence. The authors focused on the frequently occurred forms of pathomimia, which are provoked by stress factors and are transient. An approach to the treatment of skin autolesions in children has been proved and it is noted that the general treatment of pathomimia will depend on the degree of severity and manifestations of mental illness, and external therapy of psychodermatoses will lead to accelerating the regeneration of the skin lesion as well as the preventing of secondary infection.
