Medical Treatment Institutions of Civilian Health Care in the General System of Treatment of Wounded and Sick Military Personnel during the Great Patriotic War of 1941-1945

Summary. The aim of the study is to analyze and summarize the most significant organizational issues of the creation and functioning of evacuation hospitals (evacuation hospitals, EG) of the People’s Commissariat of Health (People’s Commissariat of Health) of the USSR, issues of their participation in the general system of providing medical care and treatment to wounded and sick military personnel during the Great Patriotic War of 1941-1945.

Research materials and methods. Research materials – regulatory documents governing the procedure for the creation and functioning of evacuation hospitals of the People’s Commissariat of Health of the USSR during the Great Patriotic War; scientific papers and publications, including in electronic form, on the topic of the study. Research methods – historical method, methods of generalization and content analysis, analytical method, methods of comparison and description.

Research results and their analysis. The article presents generalized data on the establishment and activities of evacuation hospitals of the USSR People’s Commissariat of Health, which functioned in the general system of medical evacuation support (LEO) for wounded and sick servicemen, and demonstrates the necessity and significance of the titanic work done by medical workers of the EG in treating wounded and sick servicemen during the Great Patriotic War of 1941–1945.

Typical (Main) Tasks Performed by the Joint Medical Teams of the Federal Medical and Biological Agency in Various Emergency Situations

Summary. The objective of the study is to define and develop typical (main) tasks performed by the joint medical teams of the Federal Medical and Biological Agency (FMBA of Russia, the Agency) in providing medical care and conducting medical evacuation of victims and patients in various emergency situations.

Research materials and methods. Research materials are regulatory, administrative and methodological documents in force in the FMBA of Russia system on the problematic issue under study; survey cards for the status, use and functioning of the joint medical teams of the FMBA of Russia in the general system of medical evacuation support for victims and patients in emergency situations, completed by highly qualified medical specialists of the governing bodies, district medical centers, medical organizations of the second and third levels of the FMBA of Russia, with extensive experience in practical and scientific work; scientific papers devoted to the creation of mobile medical units and their activities to eliminate the medical and sanitary consequences of emergencies; reports and publications reflecting the experience of the FMBA of Russia joint medical teams involved in the liquidation of the consequences of various emergencies.

Research methods: historical method, methods of content analysis and expert assessment, logical and information modeling, statistical and analytical methods.

Research results and their analysis. The paper presents typical (main) tasks, substantiated and developed using the method of expert assessments and the results of the analysis of the experience of the Agency joint medical teams involved in the liquidation of the consequences of emergencies of various nature, which are recommended to be assigned to the joint medical teams during their work to eliminate the medical and sanitary consequences of emergencies.

A proposal was made on the advisability of considering the developed typical (main) tasks for their official approval and, in case of a positive decision, to include these tasks in the relevant regulatory and methodological documents.

Organizational Models of Functioning of Territorial Centers of Disaster Medicine

Summary. The objective of the study is to analyze the current organizational models of functioning of territorial disaster medicine centers (TDMCs) of the constituent entities of the Russian Federation (constituents).

Research materials and methods. Research materials – data on the number of emergencies, the number of victims, fatalities, hospitalizations and deaths in medical treatment organizations (MTOs) in dynamics in 2019-2024 and in the context of organizational models of TDMCs over a five-year period.

Research methods – direct observation method, SWOT-analysis, analytical, statistical processing method using a special information program.

Research results and their analysis. The advantages and potential risks of various organizational models of TDMCs are presented, which, taking into account the territorial characteristics of healthcare in specific entities, are important when choosing a particular model by the regional healthcare authority; the results of the study of medical and statistical indicators in emergencies and a comparison of their values in regions with different organizational models of TDMCs functioning are presented. Proposals have been made on promising options for organizing the provision of emergency medical care in the pre-hospital period at the regional level.

A conclusion has been made on the advisability of maintaining the variability of organizational models of the TCMC, taking into account the diversity of territorial features of the constituent entities of the Russian Federation.

Application of the "Fast Cycles" Method in Training Various Categories of the Population in Providing First Aid

Summary. The purposies of the study are to determine the level of readiness of various categories of citizens to provide first aid; to identify the effectiveness of the Rapid Cycle Deliberate Practice training method, taking into account the changes made to the legislative framework in the field of first aid and which came into force on September 1, 2024.

Research materials and methods. Research materials are federal laws, regulatory legal acts in the field of first aid in the Russian Federation, methodological recommendations for first aid training.

Research methods are an analytical method, methods of questionnaires, interviews, modeling, methods of synthesis, comparison, generalization, experiment.

Research results and their analysis. The Rapid Cycle Deliberate Practice training method can be used to train persons who are obliged or entitled to provide first aid, including in the form of self-help, and is recommended for the formation of a sustainable skill along with the four-stage training method.

Impact of Medical, Social and Psychological Consequences of Emergency Situations on the Health of the Population and Rescuers: a Review of Scientific Publications

Summary. The objective of the study is to examine and analyze scientific publications devoted to the impact of medical, sanitary and socio-psychological consequences of emergency situations (ES) on the health of the population and rescuers.

Research materials and methods. Research materials – foreign and Russian publications on the impact of medical, sanitary and socio-psychological consequences of ES on the health of the population and rescuers. Research methods – analytical method and generalization method. During the study, manual searches and requests for official websites of highly rated domestic and foreign scientific journals on the specified topic were performed and the main blocks characterizing the impact on the specified contingents of medical, sanitary and socio-psychological consequences of emergency situations were identified.

Research results and their analysis. The number of emergency situations is growing all over the world, the consequences of which are becoming increasingly serious. To effectively respond to emergencies, it is necessary to know about the expected medical and sanitary and socio-psychological consequences and their impact on the health of the population and rescuers, in connection with which specialists need expert advice on issues of protecting the health of victims.

For example, emergencies such as natural disasters have a significant impact on existing chronic health problems, reduce the period of remission and contribute to the activation of the “dormant process” due to the population’s failure to comply with the rules and terms of taking medications in emergency situations due to their unavailability and difficulties in accessing medical care.

Much attention in the review is paid to post-traumatic stress disorders (PTSD), anxiety and depression – potential consequences for the mental health of victims of emergencies.

Currently, the development of diagnostics, methods and programs of preventive measures that can reduce the prevalence of primary PTSD, relieve symptoms of developed symptoms and improve social adaptation of both the affected population and rescuers are of particular relevance.

Pathogenetic Basis for Determining Resistance to the Toxic Effect of Oxygen

Summary. The aim of the study is to summarize the data contained in Russian and foreign scientific publications devoted to the study of resistance to the toxic effects of oxygen.

Materials and methods of the study. Research materials – scientific publications on the study of resistance to the toxic effects of oxygen. The search for publications was carried out using the electronic databases MEDLINE, Embase, Scopus, Web of Science, eLIBRARY, PubMed and Google Scholar for 2009-2024.

Research methods – analytical method and generalization method.

Research results and their analysis. Prevention of the toxic effects of oxygen and prediction of physiological reactions of the body of people working in conditions of increased pressure of gas and water environments or receiving oxygen barotherapy (oxygen therapy) in a pressure chamber have become highly significant. Individual tolerance to oxygen toxicity of the central nervous system (CNS) may vary slightly in the same person depending on his functional state and circadian rhythms. It has been suggested that resistance may depend on the genetically determined effectiveness of the antioxidant system, the amount of adipose tissue in the body, but there is no reliable evidence for this. It has also been reported that adaptation to hyperoxic exposure and degradation of adaptive mechanisms to it occur after a certain period of absence of such exposure, which requires further research.

The morbidity of the personnel of the duty guards of the fire and rescue units of the Federal Fire Service of emercom of Russia: according to medical examination data

Summary. The objective of the study is to identify the characteristics and evaluate the dynamics of morbidity among the personnel on duty of the fire and rescue units of the Federal Fire Service (FPS) of the State Fire Service (GPS) of the Ministry of Emergency Situations of Russia in 2019–2023.

Materials and methods of the study. The analysis of morbidity among the personnel on duty of the fire and rescue units of the FPS of the GPS of the Ministry of Emergency Situations of Russia was carried out at the Federal State Budgetary Institution “All-Russian Center for Emergency and Radiation Medicine named after A.M. Nikiforov of the Ministry of Emergency Situations of Russia” based on the data of medical examinations conducted in 2019–2023. In total, data on the health status of 2,381 people aged (34.6±0.2) years were analyzed. The survey data were checked for normality of the distribution of features, arithmetic means and standard errors were calculated.

Results of the study and their analysis. The structure and levels of morbidity of firefighters according to the results of medical examination were identical for all years of observation. The most relevant for firefighters were 6 classes of diseases: digestive organs, sensory organs, musculoskeletal system, endocrine system and metabolism, respiratory organs and circulatory system. The prevalence of diseases of these classes was consistently high throughout the analyzed period and fluctuated within 200-600‰. Reliable age and length of service features of morbidity levels were revealed. For example, in the age group of 18-34 years, diseases of the sensory organs, diseases of the respiratory organs, diseases of the musculoskeletal system prevailed, and in the age group of 35-45 years, the share of diseases of the digestive organs increased by 3.5 times, musculoskeletal system by 3 times and the share of diseases of the circulatory system by almost 5 times. It is concluded that the structure and levels of morbidity of the employees of the FPS GPS EMERCOM of Russia are influenced by age characteristics and length of service, which indicates the need to take these factors into account when planning preventive measures.

Analysis of the Severity of the Condition of the Victims with Shock-Induced Injuries Sustained in Road Accidents on the Federal Highway M-8 "Kholmogory" in the Arkhangelsk Region

Summary. The purpose of the study is to analyze the features of the severity of the condition and categories of combined injuries in victims of road accidents on the federal highway M–8 “Kholmogory” for planning further therapeutic and tactical strategies of the treatment process.

Materials and methods of research. The materials of the study are medical records of 50 patients who received shock injuries as a result of road accidents on the federal highway M-8 “Kholmogory” and were taken for treatment to the trauma center of the Arkhangelsk Regional Clinical Hospital. The research method is statistical using the STATA ver.12 program.

Research results and their analysis. Upon admission to the trauma center, the condition was assessed as severe and extremely severe in 70% of the victims. The distribution of victims according to the category of combined injuries, the severity of shock, as well as the seasonality of injury is presented.

Simulation Modeling of the Elimination of the Medical and Sanitary Consequences of a Mass Accident Using the Example of the Belgorod Region

Summary. The aim of the study is to conduct a comparative analysis of the activities of emergency medical care during medical evacuation of victims in a mass road traffic accident (RTA) before and after the creation of the Unified Center for Emergency Medical Care (EMC) and Disaster Medicine (DM) using the example of the Belgorod Region.

Materials and methods of the study. Using simulation modeling, a virtual model of medical evacuation of victims from the accident site on the M-2 Crimea highway in the Ivnyansky district on the border with the Yakovlevsky district was developed. In the course of a series of experiments, the analysis and optimization of this model were carried out. The obtained data were systematized and statistically processed using the Student’s t-test for unrelated populations.

Results of the study and their analysis. As a result of the experiment, a natural and reliable decrease in the time for medical evacuation of patients was established from (105.8 ± 12.1) min – according to retrospective data to (43.2 ± 1.18) min – according to the model data. The results of the study showed a clear advantage of the functioning of the Unified Center for Emergency Medical Care and Medical Assistance to ensure timely and correct medical evacuation of victims in a mass accident.

Providing Medical Care to Patients with Burn Injuries in the Practice of Emergency Medical Teams in Ryazan, 2019-2023

Summary. The aim of the study is to analyze the nosological structure and prevalence of burns based on calls to the Emergency Medical Service (EMS) of Ryazan.

Research materials and methods. Research materials are reports of EMS teams. Research methods are an analytical method and a direct observation method. Statistical data processing was performed using Microsoft Excel 2007 software packages with the “Data Analysis” add-on.

Research results and their analysis. During the COVID-19 pandemic, compared to other study periods, significantly lower rates of appeals to the EMS of patients with burns were noted. A change in the structure of burn injuries was established, characterized by an increase in the number of chemical and a decrease in the number of thermal burns. The timeliness and adequacy of emergency medical care for patients with burns is confirmed by their low mortality in the prehospital period.

Application of the Spectral Analysis to Assess the Complex Sensorymotor Reaction of Workers at Nuclear Energy Facilities

Summary. The objective of the study is to determine the level of psychophysiological state of workers at nuclear facilities using an integral indicator calculated based on the spectral analysis of the results of the complex sensorimotor reaction (CSMR). To compare this indicator with the standard indicators contained in the current “Methodological Guidelines for Conducting Medical Examinations and Psychophysiological Surveys of Workers at Nuclear Facilities” of 1999 (MU-1999). To determine the boundaries of its normal fluctuations, defined as two standard deviations (SD) from the mean, and unacceptable values of this indicator.

Research materials and methods. Research materials – the results of the psychophysiological examination conducted in the laboratories of psychophysiological research in medical organizations of the Federal Medical and Biological Agency (FMBA of Russia) – 20 organizations and the Ministry of Health of Russia – 12 organizations in accordance with the order of the Ministry of Health dated July 28, 2020 No. 749n. In total, the analysis included the results of 6,085 examinations performed from 19.01.2021 to 07.03.2024. Distribution of the examined by gender: men – 5,423, average age (44.5 ± 0.9) years; women – 662, average age (41.3 ± 0.8) years. Research methods – the fast Fourier transform (FFT) method was applied to the SSMR results, implemented in the form of the authors’ own algorithm in the MS Excel environment with the calculation of the “Frequency density of signal power” (Pω) indicator, normalized to the number of SSMR stimuli in the form of the decimal logarithm Lg (Pωnorm).

Research results and their analysis. The results of the assessment of the SRMT using the standard indicators “Mean reaction time” (MRT) and “Root mean square deviation” from MU-1999 were obtained and compared with Lg(Pωnorm). A stronger correlation of the Lg(Pωnorm) indicator with both MRT and SD was revealed than between these indicators. Statistically significant differences in this indicator with changes in the SRMT were established; its upper and lower limits of the norm, equal to 3.63 to 4.83 logarithmic units, and their unacceptable values were determined.

Conclusion. The study provides a rationale for the relevance of research in the field of development and improvement of indicators for assessing the functional state of the central nervous system of subjects using the mathematical apparatus of the FFT. Based on the data of 6085 examinations, a new integral indicator Lg(Pωnorm) was proposed, a method for its calculation was given, and its standard values were determined.

First Aid for Hypoglycemia: a Review of Current Recommendations

Summary. The aim of the study is to analyze international and national recommendations for first aid (FA) in hypoglycemia and, based on the results of the analysis, develop a draft universal algorithm for providing FA in hypoglycemia.

Research materials and methods. The study materials are scientific publications that present the results of original studies on the provision of FA in hypoglycemia, as well as international and national recommendations for providing FA in hypoglycemia. The analysis included recommendations published in 2014–2023.

Research results and their analysis. This paper provides an overview of current international and national guidelines on FA for hypoglycemia. Taking into account the provisions of the current guidelines, the authors have developed and proposed for open discussion a draft universal algorithm for providing FA for hypoglycemia, which determines the optimal sequence and content of measures to provide FA depending on the condition of the victim. The universal algorithm can serve as the basis for the development of a unified procedure for providing FA for hypoglycemia that would regulate and elaborate on the rules for providing FA in this emergency, as well as for the formation of standardized practices for training FA and providing FA for hypoglycemia, which, in turn, would help to increase the frequency and effectiveness of the provision of FA and reduce the mortality from this life-threatening condition.

The Relevance of Carrying Out Scientific Research on Problem Issues of Medical and Evacuation Provision in Complicated Emergency Situations - Armed Conflicts

Summary. The objective of the study is to identify and analyze the factors that determine the need for scientific justification for the use of advanced methods, innovations and technologies of medical evacuation support (MES) in complicated emergency situations (ES) taking into account the experience of the Special Military Operation (SMO) and other armed conflicts.

Research materials and methods. Research materials are regulatory and legal documents, “field reports” on the work of mobile medical units (MMF) in complicated emergency situations, articles in Russian and foreign journals on the topic of the study.

Research methods are an analytical method, methods of generalization, content analysis, direct observation, comparison and description.

Research results and their analysis. The article presents a thematic analysis of the work of the International Medical Emergency Response Teams (IMBR CR) of the World Health Organization (WHO) in armed conflicts in different countries. The key problems characteristic of the medical response system in the Russian Federation in the pre-hospital period and outside the theater of military operations (TOMO) are identified, requiring modern scientific justification and rapid implementation of scientific developments into practice. The factors determining the importance of scientific developments in the provision of medical care and timely medical evacuation of victims to specialized medical treatment organizations (MTO) of civilian healthcare are substantiated.

It is noted that, compared to other areas of healthcare, disaster medicine is an area most open to innovation and requires the use of effective, evidence-based research, especially in the area of pre-hospital medical response and the work of civilian MTO in the rear of the country.

The main areas of activity of the Disaster Medicine Service (DMS) of the Ministry of Health of Russia have been defined, the work within which is most in need of the implementation of the results of scientific research, etc.
